Usually, the miles and time quoted is from new. I have never gotten a Toyota extended warranty so I am using others like Ford to comment.
The first 2 years of Toyota Care is usually included in those quotes so deduct 2 years from your quoted years and that is what you pay for. Example 10 year quote would be 8 years extend.This my quote for a hybrid (Trailhunter)

You are correct. Toyota Care is two years worth of oil changes and tire rotation. The extended warranty takes the warranty beyond the 3/36 to whatever you would like. Personally for me, it’s a no brainer and a fantastic “insurance” policy. Something breaks after the 3/36, Toyota covers it, no deductible. You sell the car, $50 to transfer it to the new owner or they will prorate your money back. I don’t want to pay for a $1k headlight, interior screen going out or a turbo acting up. Easy decison, for me…I thought the Toyota Care was the basic maintenance they include (Oil/Filter, Tire Rotation) for two years. Then the warranty is 3 years or 36K, and 60 months Powertrain.
Maybe the Toyota offering not only does repairs out of warranty but also includes basic services?
Sadly, the "included" oil change is not 5k so I will either pay for the extra or do it myself.

No, you have to work with the dealer. There's a bit of paperwork that needs to be exchanged since you didn't buy the vehicle from them.Can you just get on that site and purchase it??
Is Toyota different than other manufacturers when it comes to selling extended warranties? For example with Honda you can buy Honda Care from the selling dealer or go check a forum and find a finance guy at another Honda dealer who sell it for a few hundred over cost since it literally takes that person 5 minutes to complete the form online. Same with Mercedes, Audi, BMW etc. There is typically a huge markup for the dealer for these warranties and is a cash cow for most.1. you should NOT post cost matrix in forums posts (especially tied to a name), it can get a dealer/salesperson in trouble with toyota usa or get them banned from selling out of state. dealers have complained to toyota usa about these people underselling them by a lot and cutting into the profit margins with 3rd party ext warranties or toyota vsa they upsell for higher costs.
2. remember toyota VSA only take effect after the oem 3/36 factory warranty
3. toyota vsa's start from date of first in-service date, so the first 3 yrs are wasted lost.
ie : 10/125k you actaully only get an additional 7yrs/89k miles out of it since it starts after 3yrs and./or 36k miles up to the stated contract amount.
